Reviewing contractor documentation across Iraqi projects, the recurring gaps are rarely exotic. They are the basics, missing or inconsistent — and they surface at the worst moment: during a client audit or tender clarification.
The usual suspects
The most frequent findings:
- HSE policy unsigned, undated or naming previous management
- Risk assessments that do not match the actual method of work
- PTW systems on paper but no closed permits on file
- Training claimed but certificates missing or expired
- No legal register — or one copied from another country
- Incident reports without root cause or closed actions
Close them before the client finds them
Each gap has a defined fix and most can be closed in days. A structured gap review against your client's requirements — with a prioritized close-out list — is the cheapest audit you will ever commission.
